"Much current medical advice is quackery," cautions Le Fanu in this remarkably engrossing scholarly study of medical progressAand the recent lack thereofAin the 20th century. Le Fanu (a medical columnist for London's Daily and Sunday Telegraph) contemplates what he sees as the unhappy situation of contemporary health care. The decades from the 1940s to the 1980s saw some of the most critically important advances Western medicine has seen, from penicillin to the heart pump that made open-heart surgery possible. Yet doctors are disillusioned, and patients are turning in droves to alternative forms of medicine. How has this dilemma come about? Le Fanu first details the astonishing breakthroughs of the earlier part of the 20th century (he describes, for instance, the progress made by the first patient ever administered penicillin). But, more controversially, he argues that since the 1980s medical progress has been crippled by two developments, which he terms "Social Theory" and "New Genetics," respectively: according to the author, misguided epidemiologists promote a lifestyle changes (low-cholesterol diet, etc.) as a means of preventing heart disease; and geneticists have misled us into thinking that their research breakthroughs can eliminate genetic diseases. Both cases have been overstated, Le Fanu contends, drawing on a wealth of scientific data to attempt to show that dietary changes have done little to prevent heart disease and that genetic experiments, despite "millions of hours of research," have had "scarcely detectable" practical results. He concludes with a plea to return to the traditional in the practice of medicineAthe relationship between doctor and patientAand to a renewal of faith in the diagnostic skill and judgment of one's personal physician. English physician and Daily Telegraph columnist Le Fanu writes a thoughtful history of the only 20th-century revolution that turned out brilliantly.During the century before 1940, people grew healthier and lived longer through improved hygiene, housing, and nutrition. Once they got sick, however, doctors weren't that much help: except for a few treatments (such as thyroid hormone, insulin, and vitamins) a patient got better pretty much on his own--or he didn't. WWII marked the beginning of a torrent of miraculous advances. To label these miracles is no hype. Dreadfully sick people received penicillin, cortisone, or lithium--and suddenly they weren't sick. Every single child who contracted leukemia in 1950 died; today almost all live. Victims of congenital heart disease or kidney failure lived as pitiful invalids if they lived at all; now they live normally. This was a wonderful period full of heroes, and Le Fanu describes it superbly in the first half of his story. Then he grows sober, thoughtful, and pessimistic. Medicine's golden age peaked in the 1960s, he writes. Important discoveries trailed off after 1970, introduction of genuinely new drugs dropped sharply, and two disturbing trends appeared. He calls one the Social Theory. Misled by triumphs of the golden age (proof that smoking causes cancer and treating hypertension prevents strokes), doctors embraced a utopian theory of prevention with enthusiasm unaccompanied by proof. Readers will be jolted by the author's claim that diet, lifestyle, and pollution contribute only marginally to ill health. Obsessive efforts to fine-tune our diet and environment (medical correctness) have, in Le Fanu's view, produced little beside anxiety. The author also takes a dim view of the New Genetics: science's fascination with DNA, genetic engineering, and genetic therapy. He points out that 20 years of expensive research, media obsession, and wildly optimistic claims have produced only minor benefits to patients. Le Fanu's doubts about prevention and genetic engineering place him in the minority among laymen as well as doctors, but he makes a convincing case in this readable and informative account. -- Copyright � 2000 Kirkus Associates, LP. Everything you know is wrong...

Very rarely, I come across a book so enjoyable that I have to put everything else on hold until I finish reading it. This is one of those books. It combines fanscinating information, intriguing analysis, and a thoroughly enjoyable writing style.
The book is divided into two major sections: an extended prologue, and the book proper. The prologue is nearly half of the book, and it describes twelve definitive moments in medicine in the past 50 years. This section is informative and enjoyable, and the book is worth reading for this part alone.
The second section is even better. Le Fanu believes that medicine experienced its Golden Age during the fifties and sixties, and is now in decline. He provides compelling evidence in support of this claim, and his discussions on the false hopes brought about by genetics, and the falsity of the Social Theory (the theory that most of our health problems are caused by environmental factors) are great, and probably largely correct.
This is a book that I would recommend to anyone. It is easy enough for the layman to understand, and I think that those in the medical profession will get a lot out of it, whether they agree with the author or not. READ THIS BOOK!!!

Good antidote to thoughtless faith in medicine

Le Fanu writes extremely well, as long as he's not ranting. His coverage of the history of western medicine is informative and highly readable, if debatable on small points. His talent is creating a real story out of a collection of facts, and delivering it in a punchy and amusing way.

Le Fanu successfully communicates his main thesis - that the history of medical progress consists mainly of accidental or fortuitous discoveries, and that it is therefore not the rational machine that many people imagine it to be. We know much less about human health than you might think, and even less about how to go about improving it. Since many otherwise intelligent and well-informed people place blind faith in doctors, medicine, and in press stories about health issues, Le Fanu is performing an important public service.

The book is marred by his rants against modern medical research. While this industry undoubtedly deserves criticism, he makes a poor job of it. He ceases to support the points he makes, and since many of them are debatable, it is difficult to take this part of the book seriously. For example, he suggests shutting down all departments of epidemiology in all medical educational and research institutions. While I hold no torch for epidemiology, this strikes me as excessive, and he fails to win me over with any rational explanation as to how this would help.

Despite my reservations about the final chapters, I would highly recommend this book. Anyone who already harbours doubts or concerns about western medicine will find the book interesting. Anyone who does not really needs to read this.

Provocative and infuriating survey

The Telegraph's medical columnist claims that medicine's golden age was from 1945 to 1980, due to the chance discovery of drugs, advances in clinical science and innovative technology. He believes that it is now exhausted, and laments that the vacuum is being filled by what he thinks are the dead ends of New Genetics, epidemiology and social medicine. It is untimely to write off genetics when the Human Genome Project offers such exciting possibilities.
He calls for more research into the causes of disease, and rightly rejects idealist explanations. Doctors used to think that peptic ulcers were due to `stress' or `personality', but in 1984, Barry Marshall, a young Australian doctor, identified a type of bacterium that triggered them. A seven-day course of antibiotics was the cure. The same organism caused two-thirds of stomach cancer cases. In 1986, Thomas Grayston discovered that the bacterium chlamydia caused heart disease. Perhaps as yet undiscovered bacteria cause arthritis, schizophrenia, leukaemia, MS, diabetes and ME.
He has a brilliant chapter on how the use of new drugs refuted Freudianism, as chlorpromazine effectively relieved schizophrenia's symptoms, lithium mania's, prozac depression's and valium anxiety's.
Le Fanu shows that the influential historian of medicine Thomas McKeown wrongly denied doctors the credit for tuberculosis's decline. Doctors' seclusion of TB patients in sanatoria dramatically reduced the infection's incidence.
He argues against social medicine, rejecting all social and economic explanations of illness. But lifestyle changes - losing weight, improving diet and exercising more - do prevent diabetes and promote health and well-being (British Medical Journal, 14 July 2001, page 63.)
He claims that medicine has run its course. We have seen the misanthropic idea of the end of history, of politics, of industry and of class. Now Le Fanu pronounces the end of medicine. This is a provocative and infuriating book, full of ideas and prejudices. We need the tests of practice to see what he has got right.

Gr 1–4—Folktales involving water abound in all cultures, but this tale is unusual in using water as a metaphor for story: just as we need water to nourish our physical selves, we need stories to feed our spirits. In Turk's fable, a lone storyteller remains in a Moroccan city where the water sources have all dried up. When a young boy seeks water, the water-seller has only a bowl to give him, but the storyteller tells him a tale that miraculously fills the bowl. In a series of nested stories, the boy's thirst is quenched, and by retelling the stories Scheherazade-style to a sandstorm in the form of a djinn, he is able to save the city and also replenish its water supply. In predominant hues of brown and blue, Turk's bold, semiabstract mixed-media illustrations conjure up swirls of sand and waves of water, evoking the environment and its people. The spreads contain concentric borders representing each of the stories as it is told. Using age-old literary elements and a loose, contemporary art style filled with symbolism, Turk successfully melds two equally important concerns of our time—the need to keep storytelling alive and the need to protect and conserve our drinking water. A future classic

I just received my copy and was immediately spellbound by the story and illustrations. Turk is masterful in both illustrating and storytelling! It is a richly metaphorical tale, which brings the reader deep into the world of Morocco. It made me long for simpler days where the world had more pauses and reflective time. This book is a gem, and I can't wait to share it with my 4 year-old when he wakes up!

“When a storyteller dies, a library burns.”

Credit the internet age for doing what the television age never could. I don’t know if you’ve noticed, but there is a movement around the world that can be interpreted as nothing so much as a direct response to our digital age. You may have noticed it in small things, like the rise of Steampunk or the sudden surge of interest in Maker stations and the kinds of “hacking” that look suspiciously similar to activities found in shop class in days of yore. All this comes about because people have come to believe that we do not create enough tangible objects in our day-to-day lives anymore. And while this is true, let us not forget that we do not create enough intangible objects either. I’m talking about storytelling, that ancient artform that is currently seeing a worldwide resurgence. It isn’t just the increase in storytelling festivals and podcasts like The Moth here in the States. Young people in countries worldwide are doing what their elders have desired for decades; they’re asking to be told a story. Taking his cues from the newfound interest of young Moroccans in Marrakech in the ancient storytelling tradition, author/illustrator Evan Turk uses the folktale format to craft an original story about storytelling, weaving, history, and language. The end result is a twisty turny story within a story within a story that challenges young readers even as it lures them in.

Once, in the great country of Morocco, storytellers flourished and the cities’ fountains flowed with cool, clear water. As time went on the people became comfortable and forgot about the storytellers, and so they disappeared over the years. So too did the fountains dry up, until one day a boy went looking for some water. What he found instead was an old storyteller. As the man told his tale he would end his story with a story within a story and the boy would find his brass cup filled with liquid. Even as this was happening, however, a desert djinn saw the drought as an opportunity to reclaim the cities that had previously held him back with their fountains. Yet when the djinn was set to level his town, the boy managed to delay him with his storytelling. And as he wove his tale, the people were able to refill their fountains until finally storytelling and water ran freely in the cities once more.

My brain is not what it used to be. Remind me again. What’s that term for a story that tells a story that tells a story that ends only when the innermost story doubles back and each tale is finished in turn? Is there a word such a thing? I suspect that the storytellers amongst us would know. The most obvious similarity to this book that comes to mind is, of course, the tale of Scheherazade. Indeed, the boy uses his stories to trick the djinn. And what could be a more natural comparison? In both tales it is storytelling that proves to be the saving of us all. Our thirst is quenched and we are tied to our history like never before. The obvious question then is whether or not Turk’s text is too complex for kids to follow. Sure, he distinguishes between the tales with different colored fonts, but will that be enough to allow them to remember what came before as they plunge deeper and deeper into the narrative? I think there may be some confusion at work, certainly. I wouldn’t necessarily hand this to a three or four-year-old. However, Turk’s text takes pains to remind the reader where the tale was before. The art helps as well. Confusion, such as it is, will be held to a minimum.

I first knew of Turk’s work when he illustrated Bethany Hegedus’s Grandfather Gandhi. In that book he integrated real spun cotton threads into the art, knowing full well the importance spinning had to Gandhi and his followers. In this book, weaving is the craft of choice so I wondered, not without reason, if woven threads would make their way into the art. As it happens, there are plenty of water-soluble crayons, colored drawing pencils, inks, indigo, sugared green tea, and even art created by heat gun and fire in the illustrations, but nothing so simple as thread. Turk mentions this on his publication page and he puts a little note to the reader there as well. It reads, “Look for a blue glimmer of hope to appear around each story!” and a small blue diamond appears. Naturally, I was curious so I looked. Sure as shooting, after each story’s text a diamond appears. However, as the stories appear within stories within stories, the diamonds grow more elaborate and decorative. Then, as the stories end one by one, the diamonds simplify once more. I began searching the art for more diamonds and here Turk doesn’t disappoint. If you look closely at the borders of the book, you see that the diamonds appear when there is hope and fade from blue to brown diamonds when hope dries up. As the storytelling increases the borders fill in more and more blue, just as the townspeople fill their fountains with bowl after bowl of water. Point out to a child reader the diamond motif and you are sure to be surprised by all that they find hidden in these pages.

I should probably say something about Turk’s art itself. When I reviewed Grandfather Gandhi I had difficulty putting into words precisely what Turk does with his images. So I looked at the book’s professional reviews. His art causes reviewers to use terms like “dynamic visuals”, “stylized” and “strikingly patterned”. They say his art displays “bold, expressive imagery” or that he “mixes carefully detailed renderings with abstracted expressions of emotional struggle.” I agree with all of that but no one mentions his faces and hands. The patterns here are striking and upon closer inspection they yield such marvelous details it wouldn’t take much for this art to spin wildly out of control, opting for an abstract approach to the proceedings as a whole. Instead, Turk centers his art through the hands and faces of his characters. Look closely and you’ll see what I mean. The old storyteller’s hands are gnarled and wonderfully expressive, even as his audience of one clutches a single brass bowl. The hands of a cunning neighbor stroke her child as she schemes, while a princess, escaping on the night before her wedding, holds up her hennaed hands in despair. Hands. Heads. Hearts.

There’s been a lot of discussion lately about diversity in children’s literature. Specifically, some of that discussion has concerned those books written by white people about other cultures. It’s not a new phenomenon but what is a bit new is the increasing understanding that if you are going to use another culture, you need to do your homework. If, for example, you are setting a story in Morocco, then you need to make the readers understand why you made that choice. That it wasn’t arbitrary. This is yet another of the many reasons I’m so impressed with Turk’s work here. That he sets his story in Morocco (contemporary Morocco, by the look of it) is deeply purposeful. The Author’s Note at the end explains further. From this we learn that Morocco’s public storytellers or hlaykia have told tales for “nearly one thousand years” and yet “Only a handful of master storytellers remain”. All is not lost, though. Renewed interest in storytelling has surfaced, specifically at a restaurant called Caf� Clock in Marrakech. Turk then closes with a small Bibliography of sources on everything from storytelling to carpet weaving. The book then is not an appropriation of an “exotic” culture done on a whim but rather a considered, thoughtful selection that serves as an ideal setting for a tale about storytelling then, now, and in the future.

It was once part of a children’s librarian’s training to know how to tell a story from memory. Here in America it was even considered part of a children’s librarian's heritage, though in the last few decades it has been fast forgotten. There are still pockets that remember, though. That’s why books like Turk’s give me the oddest little sense of hope. As I mentioned before, storytelling everywhere is seeing renewed interest. It seems odd to say, but this book, wrapped as it is in classic motifs and themes dating back hundreds, even thousands, of years, is one of the freshest, most timely picture books I’ve had the honor to read in a long time. Visually stunning with a storyline to match, Turk is beginning to make good on his talents. This is a man with storytelling in his blood and bones. Our children reap the rewards. A can’t miss book.

The power of stories

Summary: Many years ago, in Morocco, people were sustained by the cool water of their fountains and the refreshing tales of their storytellers. As life got easier, though, the storytellers began to die out; at the same time, the fountains started to dry up. A young boy goes in search of water. He finds a storyteller perched on the edge of a dried-up fountain. When the storyteller begins a captivating tale, the boy finds his cup is full of water. Each part of the story ends with a cliffhanger, and the boy returns each day until the fountain is full again. When a djinn from the Sahara desert threatens to destroy the drought-stricken city, the boy becomes the storyteller, keeping the djinn at bay until the people of the town unite to fill all their fountains and save their city. An author’s note tells of Morocco’s storytellers, whose traditions have been being replaced by TV and the Internet, and a recent move to preserve their art. 40 pages; grades 1-5.

Pros: A beautiful tale that explores the power of stories in humans’ lives. The unique blue-and-gold themed illustrations may put this book on the Caldecott contenders’ list.

Cons: While this has the feel of a folktale, it is not a traditional story. The publisher calls it “an original folktale”. A bit difficult to classify the genre.

This book is a gem because it simplifies the most complicated concepts so that students can use qualitative methods for research effectively. I couldn't have written my dissertation without this text and it's coaching style. I recommend it to anyone who wants a non-jargon approach to research design and analysis.

About the Author

Robert M. McManus is the McCoy Associate Professor of Leadership Studies and Communication at the McDonough Leadership Center at Marietta College in Ohio. He co-edited Leading in Complex Worlds (Jossey-Bass) and has served as the Chair of the Leadership Education MIG for the International Leadership Association.

Gama Perruci is the Dean of the McDonough Leadership Center at Marietta College in Ohio. He also serves as a consultant for The New York Times (nytimesinleadership.com) and leadership session facilitator for Dartmouth College’s Rockefeller Center for Public Policy and the Social Sciences.

A very good instruction manual

This book describes in clear detail how to properly bind a book for maximum durability. It includes details on case bindings (the easiest and weakest), library bindings (one of the strongest), flexible and sunk-cord bindings (traditional), and limp bindings. Although the author makes much about having the right tools, his descriptions are clear enough that one can improvise. With this book, I was able to make a book from paper, boards, and leather that came out quite well on my first try.

A "Must Have" Book for the Serious Bookbinder

Of the many popular books in print today on bookbinding, this one is by far the most comprehensive. It reads like a graduate level textbook and must be studied carefully; it isn't conducive to casual reading. Many of the sentences are difficult to understand and must be thought about for awhile to figure out. Nevertheless, I wouldn't want to undertake a new step in the binding process without first reading what this book has to say. I strongly recommend it for the serious student of bookbinding.

Superb reference book

This book definitely is not for a beginning bookbinder. It is, however, a superb reference book. It discusses, in detail, a vast number of topics -- some not discussed by many other books on the subject -- in a very readable style. This is one of the most useful, and most referred to, books on bookbinding that I own.

I'm glad I was able to buy a used copy because ...

the content of the book is required for dental school. I'm glad I was able to buy a used copy because it was much more expensive at the school bookstore. The writing is obviously more scientific, as it is a textbook, but the messages are mostly conveyed in a clear, concise manner. This is one of the books I'll be keeping to look back over even after I graduate from dental school because I think the material is valuable and will still apply.

About the Author
Dr. Donald McLusky was for many years a Senior Lecturer in Marine Biology at the University of Stirling, Scotland, UK. He also served as the Head of Department and Vice-Dean of the Faculty of Science. He holds a B.Sc. degree in Zoology from the University of Aberdeen and a Ph.D. degree on estuarine eco-physiology from the University of Stirling. Whilst at Stirling his research interests were centred on the biology of the Forth estuary, with many studies of the intertidal fauna including the annelids, molluscs and crustaceans. For over 25 years he was responsible for the monitoring of the effects on the intertidal environment of a large petro-chemical complex at Grangemouth on the Forth. He also studied the impact of two marine oil terminals on the surrounding marine environment. He supervised the Ph.D. programmes of many young estuarine and marine scientists both at Stirling and also in Sri Lanka, Bahrain and Portugal.

Away from Stirling University he was a Visiting fellow at the University of Copenhagen's marine laboratory at Helsing�r on several occasions, as well as a Visiting Professor at the University of Paris, and Rivers State University, Nigeria. He was the Honorary Secretary of the Estuarine and Coastal Sciences Association (ECSA) and remains their Trustee. He was a member of the Council of the Scottish Marine Biological Association, and a member of the Scientific Advisory Committee of Scottish Natural Heritage. He is the author of over 60 scientific papers on marine, estuarine and freshwater biology.

He is the author of the well-established textbook The Estuarine Ecosystem, which is now in its 3rd edition, as well as Ecology of Estuaries, and the editor of several volumes including The Freshwaters of Scotland, North Sea - Estuarine Interactions and The Environment of the Estuary and Firth of Forth. He was the editor of the journal Estuarine and Coastal Shelf Science for 10 years, from 2000.

Professor Eric Wolanski is an estuarine oceanographer at James Cook University and the Australian Institute of Marine Science. His research interests range from the oceanography of coral reefs, mangroves, and muddy estuaries, to the interaction between physical and biological processes determining ecosystem health in tropical waters. He has 370 publications and reports. Eric is a fellow of the Australian Academy of Technological Sciences and Engineering, the Institution of Engineers Australia (ret.), and l’Acade�mie Royale des Sciences d’Outre-Mer. He was awarded an Australian Centenary medal, a Doctorate Honoris Causa by the Catholic University of Louvain and a Lifetime Achievement Award by the Estuarine & Coastal Sciences Association. Eric is an editor-in-chief of Estuarine, Coastal and Shelf Science, Wetlands Ecology and Management, and the Treatise on Estuarine and Coastal Science.

Eric is a member of the editorial board of Journal of Coastal Research, Journal of Marine Systems, Ecohydrology and Hydrobiology, and Continental Shelf Research. He is a member of the Scientific and Policy Committee of the Japan-based International Center for Environmental Management of Enclosed Coastal Seas, a Visiting Professor at the Chinese Academy of Sciences, and a member of the College of Experts of the Australian Research Council.
